That is a remarkably petty reason to block something. That's when you point out to the campaign that there was an error and let them fix it. It's not like they submitted the signatures for shits and giggles and never planned on running; this was a paperwork error.
IIRC the deadline was the 2nd, so they might be unable to correct any errors. I also infer from the Ohio officials comments that it wasn't that the campaign neglected to include one form, it was that they neglected to include a statement from the candidate with each set of signatures.
If that's the case, it's not an easy thing to fix. They would have to re-gather ALL the signatures.

That's an important difference. If it's lists of signatures without any context... Yeah, that's much less reasonable.
