I used CityMapper when I was in Portugal in 2019 and it was invaluable, but mostly just for walking directions + public transit. I don't think I would ever trust it or Google Maps for bike...

Honestly I don't know what the best app for just bike directions would be. If you have time you're better off looking at somewhere like Garmin or Strava to see their heatmaps and create your own route based on those.
From personal experience I've had a similarly bad time with Citymapper, but since switching to cycle.travel I've found it really good for safe routes (at least in london). I'd highly recommend it.
