I guess the question I have is, where is the line where you don't ask for an apology and just dismiss a person from the party.

Lee Anderson, former Conservative deputy chair, has been suspended from the party after refusing to apologise for claiming that Islamists had “got control” of London and its centre-left mayor Sadiq Khan.

Essentially that's what's happened I believe, him being suspended essentially means he's been kicked out. He'll now sit as an independent in the House of Commons and receive no Tory campaign...

Although, my moneys on him being welcomed into Reform, a far-right populist party that formed from the ashes of UKIP once Brexit had been achieved. Whether he retains his seat though anyway in the upcoming election is another matter. The Tories are up for a complete trouncing, with Labour, our centre-left party, expected to win a sizeable majority.

I think the person you replied to was asking why he only got suspended after refusing to apologize, rather than for the behavior itself regardless of whether he apologized or not.

Ahh, of course, so they were, my mistake. I'll leave my comment up anyway for context.

What does this mean, in practice? What changes for him?

That means he is no longer a Tory MP and will sit in parliament as an independent unless the decision is reversed.

Further statements he makes no longer imply some credibility within the party. He will not receive Conservative Party funding and support in the upcoming General Election.
