Smallpox-like viruses exist and can mutate in the wild. The smallpox genome has already been sequenced and could be resynthesized, with current technology.

The good news is, we now have the technology to stop an outbreak, and enough has been learned from smallpox that we can treat similar emerging viruses.

What we can't stop is people mad enough to create and release species-murdering weapons.
