    That was a fascinating story. Sad that the Captain was emotionally unable to pilot commercial planes again. The manufacturer definitely had some responsibility, as did the people responsible for maintenance systems at the airport.

    For similar stories of errors and failures in a variety of fields, I can recommend the book Being Wrong, Adventures on the Margin of Error, which is one of my favorite generalist nonfiction books.
