I've had it, but never needed it. I've been in emergency rooms in Vietnam, New Zealand and Scotland. Never paid a dime, they just waived me off and said don't worry. I imagine it would only be useful if you needed an emergency flight to another country for treatment, but I only get that sort of coverage if I'm going someplace with truly third world emergency care.
