I work in the rail industry in Canada. It's not as bad as in the USA but its crazy. One incident I remember was a carman catching a piece of missing flange on a wheel and stopping the train as it was leaving town. He wasn't disciplined but his boss basically told him off for causing delays. A missing bit of flange like that inevitably leads to a derailment... that guy saved the company millions of dollars.
