Side note, one thing that Hacker News generally does that I think is a very helpful for maintaining context, is to put the original link in a comment explaining why the link was changed.

Yeah, that's generally why I state my reasoning in a comment before I change links, and also often ask the submitter if they're okay with me doing that too. AFAIK there is no real formal policy...
