Or you know, get a real DNR. Host it on a website somewhere and tattoo the URL link instead. Or contact information for your lawyer. Or get this on a bracelet, or an insert you keep in your wallet...

I've seen medical alert tattoos for diabetics who prefer it over the bracelet.

Yeah, the best thing to do is to contact a lawyer and get this done the right way. Unfortunately, it won't do much if you are injured away from home.
depends on how complicated you want your advanced directives to be... but it's also probably not a bad idea to knock out a will while you're at it.
In my area a durable DNR has to be signed by a doctor to be valid.
