The chain of events:
Quin finishes nightmare dungeon with the zappy lightning + bubble mechanic
he is standing inside the invulnerability dome, with a cheat death elixir active
he starts teleporting to town, the animation finishes
he goes to loading screen
the game is stuck there for about 2.5 minutes, after which he closes it via task manager
he logs back in to a dead character
The current theory is that the invulnerability dome disappeared after his tp went off, but the zappy lightning effect of the dungeon kept going. He's playing a druid, he had full HP and a cheat death elixir. I'm fairly sure the zap wouldn't even 1 hit him, but even if it did, it would take at least 2 hits due to the cheat death elixir.
This combined with the recent lags, server crashes and whatnot is quite concerning. I've always been a hardcore player and can deal with losing a character, but dying like this would probably make me quit the game for a while. Not a great look for blizzard, as I'm fairly certain they don't restore characters, even if they died to lag or a bug.
And we're soon getting that on Classic WoW! Seriously, I'm excited because it will be an event, but unofficial WoW Hardcore actually accepts death appeals... If you're recording, it's reasonably safe.

That's pretty cool. Probably would be too much work to implement officially.
That's a good idea
Oof that sucks. Would discourage me from playing hardcore ever again, and is from even starting.
