The big issue with smoking is doing it too much once a substance addition sets in. I've had several actual doctors (my own physicians) chuckle and dismiss once a week smoking as relatively harmless. They wouldn't even write me down as a 'smoker' on their forms at that pace. You only get into real trouble when you're smoking daily. Once a week can't even paralyze your cilia, but daily will.
So, if you fancy recreational smoking, make it a once a week (at most) affair. Also, get a hookah, it tastes a lot better and it's smoother on your lungs.

From what I’ve heard smoking, in the societies where it was traditional, usually didn’t involve lighting up and sucking down the smoke. They usually either put it into a fire in an enclosed space, sort of like incense, or they smoked through very long pipes (to cool the smoke down and limit harshness/tissue damage) and didn’t really inhale. The inhalation became a thing once the pharmacological properties became more valued than the ceremonial, social, or ritualistic properties.
Even with pipe and cigar smoking inhalation isn’t typical and, for most people without huge nicotine tolerances, might actually make you sick to the point of vomiting and hallucination. I learned this the hard way when someone handed me a cigar on the night of the 2008 election.

Isn’t putting smoke into your lungs always carcinogenic?

Even without inhalation, pipe tobacco and cigars can cause cancer on the mouth, tongue, and throat.
Salvia Divinorum: Worthwhile experience, never again.
