The emphasis is more on having a community with certain shared norms and values around engaging in productive discussions on substantive topics rather than having it be a free-for-all full of memes, shitposting, and willful trolling.
So that's a pretty open door policy really. Whatever "exclusivity" there is would largely come from naturally filtering out people who would want to find and post viral or low-effort content.

The userbase is otherwise a mostly self-selecting population from at least somewhat privacy-conscious people, which sadly skews heavily into the white-male-techy. Not inherently bad, but definitely does not foster diversity.
I don't know how much of the skew comes from privacy consciousness, I think it's probably like 70% "founder effects" (and feedback loops deriving from them) and 30% everything else.

If Deimos is interested in expanding Tildes, he might need to make membership/readership more public facing than through private one-to-one invitations, that makes for more 'organic' growth, but not necessarily quick or large growth.

In fact, I have only ever seen him suggesting the opposite; having mentioned many times over the years about how he would eventually like to open up registration at some point.
As to your main question about Tildes being exclusive. No it's not "exclusive" (other than the "don't be an asshole" requirement), nor should it ever be, IMO. I have given out several thousand Tildes invites over the years, and the only time I have denied invite requests was when the user was an obvious troll or bigot. That hardly makes everyone I have invited here the "best of the best". All it does is raise the basic requirement for joining here to "don't be a totally obvious asshole".

Don't act like an asshole and routinely make other people's experiences—or lives—worse. Almost all of the restrictions on how you can use Tildes are just more-explicit versions of this basic guideline. In general, as long as you treat others with basic civility and try to contribute in good faith, you will be welcome on Tildes.

This has both positives and negatives- on the positive side there is in-depth discussion on a variety of topics and people who are repeatedly found to act in bad faith get banned/removed. On the negative side the lack of diversity often leads to certain minorities becoming frustrated or upset and leaving based on how they are treated and the distinct lack of safe spaces and the presence of edge-case users who push boundaries of bad faith but only on specific axes often related to their privilege or general disposition.
