I’ve been taking a stack of meds for a couple years now and they work great for me. It’s Prozac, Wellbutrin, and Vyvanse.

But I’ve been feeling numb recently. I told my psych, and we’re going to trial letting go of the Prozac. What should I expect? I’d like to have food taste good again, but I also don’t want the depression to come back.

My psychiatrist says that I should reach out to her if I feel intense multiday sadness, but are there other signs I should be looking out for?

I guess I’m just trying hear more about other people’s experiences with quitting/tapering off antidepressants.

    4. The road to non-directed kidney donation

I've decided to give up my kidney to a stranger and document the process and hopefully my posts can be the kind of resources I wish I had when I first looked into this.

Someone here recently linked to Scott Alexander's post on his non-directed donation experience [1] and afterwards I got recommended Jesse Eisenberg's interview on his experience [2]. I think this led to me thinking about the subject and deciding that I at least need to look into this and see if my worries about it are founded. It turns out they are not. Here are the highlights of my research:

      1. Compared to other activities, donating a kidney is not especially risky [1]
      2. It is entirely free; all aspects of the process are free including travel [5]
      3. If anyone in your family needs a kidney, they are at the very top of the list for a living donor kidney [3]
      4. NKR will cover up to $2000/week in lost wages using Donor Shield [5], though if your work allows you to take paid time off, I highly recommend you save their resources for those without that luxury
      5. Non-directed donors can use "chaining" to have their one donation result in 3-7 further donations due to matchmaking on the NKR (this is complicated to summarize, ask if you want information or see the link) [4]

Step 1: Reach out to the National Kidney Registry (NKR)

The ideal way to become a non-directed donor is to reach out first to the NKR. They run the family voucher program that protects your family should they need a kidney in the future. They first have screening questions, then there is a questionnaire about your medical history that you complete, and immediately upon completion, they have labs ready for you to take. I reached out to NKR yesterday morning and had labs completed yesterday afternoon at a local clinic.

As for what's next, my labs will come back and the NKR says I will then choose a preferred donation center. I have absolutely no idea what considerations matter when choosing the best donation center. I assume something local, since I can't fly immediately after surgery and the drive home will be painful.

If you've considered kidney donation and didn't do it, what held you back? I want to know to 1: see if I can correct a false belief you have about the process or 2: see if I should reconsider this whole thing. Also, if anyone wants to join me on this journey, click on any NKR link and click the donate button and let's do this!

I want to donate my kidney (not to anyone in particular, which would make the process far more straightforward), but I need help navigating the bureaucracy. Here are things I have seen about living kidney donation online:

      • You can select five people to get priority access to kidneys if they need it (e.g., parents, spouse, kids)
      • They will fly you to where you need to go for screening
      • They will cover lost wages during recovery

Now I don't know who "they" is, or how to go about getting answers to these questions. Since I'm not tethered to a particular recipient, I can choose wherever to sign up, and I want to make the optimal choice both for myself and the recipient.

If you are a donor, especially an anonymous donor who's navigated this stuff, please reach out.

(warning and TL;DR : long and kinda ranty - I do want advice but this also ended up being me venting about my frustrations with two separate medical professionals)

I've suspected most of my adult life that I have some form of ADHD; some mornings I face debilitating initiation paralysis that causes me to be up to 2 hours late for work; I forget conversations happened and my mind is in a constant jumble of starting 5 tasks and finishing neither; sometimes I hyperfocus and sometimes I lack any focus - seemingly at random - and other tiny tidbits that upon a cursory glance through medical material, scream ADHD to me.

I've learned to cope with most of it, and thankfully I have a pretty chill job that lets me be flexible with my schedule without issues, but when you look at everything in context, it's pretty clear that my quality of life could be so much better if I sought professional guidance and medical attention.

So I did just that; went to my clinic, scheduled an appointment with a psychologist, and I dragged myself there.

I did my best to be objective and factual about my behavior, I made notes of stuff I did and symptoms I experienced over the course of a week, and answered every question as openly as possible, and yet everything felt wrong.

The psychologist didn't see ADHD in me, and instead chose to pursue my childhood and familial history, narrowing down on signs of anxiety. That felt viscerally incorrect to me, as it didn't reflect how I perceive my behavior. The way I understand anxiety doesn't align with how I think and behave. I don't worry about things when I am stuck in bed - I am pleading with my body to let me move so I can do the things I enjoy. I don't dread going to work - I want to go to work, and my brain says no. That is not anxiety, no matter how you frame it; at best, any signs of anxiety I may have are a byproduct of my struggles with executive dysfunction.

At the end of the session she recommended I return for a few more sessions so we can build a proper profile of my background and identify what we need to work through. But before that, she mentioned I could also see a psychiatrist, and ask them to refer me to her so the sessions could be paid for by national healthcare (I'm Romanian, for context).

So I did that, booked an appointment with a psychiatrist that seemed alright, and I basically hit the same brick wall I did before. My issues aren't neurological, they're behavioural - and I just need some counseling and discipline. And my inability to make my body move in the mornings could be just a sleep hygiene issue.

You've all heard or read about women having debilitating period pain and just being told to drink water or eat healthier or maybe go for walks more often, right? This felt like that; I'm facing a clear disconnect between my brain and my body, I have my daily life disrupted by things that are 100% out of my control, but apparently I just need discipline and better sleep. I don't buy it, as much as I want to.

I got so frustrated during the session that I started involuntarily masking and going along with the motions just to have it over. Internally I was on the verge of tears but I put a pretty smile on and left the room upbeat. That is not normal. I need help.

But they just don't seem inclined to want to offer it. I am a firm believer of Occam's razor but the psychiatrist's conclusion didn't feel like the simplest one - it felt like a massive oversimplification.

I did get a recommendation to take the DIVA-5 test (because neither of them were qualified to do it) so now I'm searching for a psychologist that is certified (which are rare, and pricey, from what I can see).

But until then, I just feel disappointed, misunderstood, and honestly quite angry. I asked for help and was given what amounts to scraps. My lived experiences were invalidated in front of me, in the places that were supposed to validate them and guide me towards finding an understanding of my behaviors and my mental health, twice in a row.

Those of you who got diagnosed, how many tries did it take? Is this the norm, just hopping from clinician to clinician until you find one that clicks and feels right? Or did I get massively unlucky?

Also, has anyone else taken the DIVA-5? How did it go for you?

I'm in my mid 20s and have had a pretty lonely life. I had a very lonely childhood, a less lonely but still fairly lonely adolescence, and again a very lonely time in college (in that case, because of a mix of mental hangups, the pandemic, personal circumstance that was a major drain on my time, and not managing all those things as optimally as I could have). At this point in my life, I essentially only have one friend, who I met a couple years ago and has since moved across the country.

I have more time on my hands now, am probably more socially adept than I used to be, but I find myself having lost or losing the drive to make friends. I guess I've gone so long without friends that I've lost sense at an emotional level of what the point even is. It's also friggin hard to meet people in the adult world and it seems like you have to put twice the energy to meet people that you vibe with half as much as when you were younger. I want to want to make friends, but it feels like I'd be setting myself up for more pain and disappointment by putting myself in that headspace. It also doesn't help that I'm pretty socially picky (not in a judgmental way, but I'd prefer a small circle of people I'm really close with over a large group that I'm not) and that I have a strong need to feel like the friends I make aren't "arbitrary", that they are the ones I am "meant" to be around. Has anyone experienced something similar or have any advice?

I had been having pain in my "upper butt" when sitting for a few weeks, so I went to an orthopedic practice that has urgent care walk in hours. He diagnosed it as sacroiliac inflammation and gave me an anti-inflammatory (meloxicam) and a PT script. The PT did not have an opening until week after next, but the medicine has has really helped with the pain.

Them took xrays during my visit, and the side-looking one shows spondylolisthesis (one of my lumbar vertebrae has slipped forward). He pointed it out (casually) and didn't say much about it other than he thought it wasn't the cause of my pain. Maybe he thought I already knew about it?

I have been having lower back pain off and on for a long time. I went to my doctor one time, and he diagnosed it as a bulging disc. He had given me some meloxicam and strengthening exercises to do at home. Since then, I would have pain in my lower back intermittently as well as occasional numbness in my leg, but I had just assumed it was a disc and tried to keep strengthening my core.

I am overweight (280lb) and not very active (software engineer). I started a new job that is in office this year, so I'm a lot less sedentary than I was working from home. I have a fantastic yoga teacher, and I do that every week. My practice in between classes has been intermittent at best, but I am trying to make it more regular.

Right now, I'm waiting for the PT to start, and I have a followup appointment with the spine specialist at the ortho in a fee weeks.

My biggest problem is that whenever I think about the xray, and seeing that disc so misaligned, my anxiety really spikes to 11. I get the "heebie jeebies", the same creepy shivering feeling I get if a bug unexpectedly crawls on me. It's happening most days. I feel kind of betrayed by my body.

I have talked to my therapist about the anxiety, and we are working on it. But in the mean time it is very stressful and distracting.

If you made it this far, thank you for reading. I am not sure what I'm really asking for. There's plenty of information online about the treatments. I guess I am wondering if other people went through it, how did it affect your mobility, quality of life, etc? Are there things you wish you had known?

Honestly, I think I could just use a (virtual) hug.

Apparently my private health insurance covers pharmacogenetics testing to find the best ADHD medication. What is it, is it legit? What's the company going to do with my cheek swabs? How do doctors feel about me going up to them and say hey internet pharma bro tells me I should be taking this instead of what your years of medical school thinks.

What I can't figure out is how my insurance company benefits from this. I'm not paying extra for this service so I must be the product right? Here's part of the marketing FAQ info from my insurance provider:

Personalized Medicine uses advanced pharmacogenetic testing and pharmacist assistance to find the most effective medication for you.

If you’re starting a new medication for a mental health condition (such as anxiety or depression), chronic pain, attention deficit hyperactivity disorder (ADHD), or neurological conditions – or taking a medication for one of these conditions and it’s not working or causing side effects – Personalized Medicine could help. It’s offered through your extended health care benefits plan, so the cost of the test may be covered. To see the list of drugs tested, click here.

It involves a pharmacogenetic test. This is a type of test that identifies how you may respond to medications. Your test results are only for you and anyone you give consent to share them with, such as your doctor. The test is run by Personalized Prescribing Inc., a Canadian company. Manulife

(Forgot to mention this is for a Canadian employment extended health insurance.)

My hypoglycemia issues are not related to diabetes fwiw. That said, I tend to get hypoglycemia a few times a day. If I catch it quick enough and treat, it's usually not a big deal, but if I get too low (maybe once I get into the 50s mg/dl), then after treating (usually about 15 minutes later), I get so. freaking. tired. Like, barely able to stand up exhausted. Currently dealing with this as we speak, and it's very frustrating. My endocrinologist told me it's normal to get tired like this while recovering. I'm curious if anyone else deals with this? If so, do you have any advice for dealing with the fatigue?

Tildes might be too small of a platform for this. If no one deals with hypoglycemia here, please feel free to remove it. I thought with the prevalence of diabetes, it would be likely there are folks who encounter this.

EDIT: In case anyone ever stumbles on this, turns out it was an insulinoma. I finally had surgery. Don't give up hope, answers can be out there, even if you feel like there's no where left to look.

Some of you may remember me from another thread, where I hemmed and hawed and fretted my way through trying to navigate my struggling relationship. I thought I'd reached the final chapter, I moved out and established a place for my child and myself. I cut as many ties as I could with my now ex, but of course several threads still remain.

Yesterday (April Fools of course) I found out that they had been cheating on me since last May, at least. Potentially longer. Of course, I'm devastated. I knew they had found someone else already recently, but I thought it was in the last month or two. This new piece of the puzzle has solved so many unanswered questions I've had, while also creating many new ones.

I feel used, financially/emotionally abused, gaslit, and more. I'm still working on processing everything. I'm trying to remain calm for the sake of the kids (especially mine) but it's really, really hard. I know it isn't a reflection on me - I tried everything I could to fix things. It is hard however not feel that utter sense of betrayal in my gut and soul like a knife.

What resources, reading, supports, guides are there for this? I'm just trying to get through today and then I have a three day weekend to work on processing this all.

I’ve gone a bit too deep on a rabbit hole after an offhand comment about protein intake and how much protein I should actually be consuming. It turns out that the 1.6g/kg of body weight is fairly arbitrary and body weight itself is not a particularly good point to use for an estimate if you are overweight. With that in mind I have been wondering about getting a DEXA body composition scan. It would be useful, I think, because it can also tell me about visceral fat which is an area I am particularly concerned about.

It turns out that it’s pretty cheap to get done; about $45 if you sign up for quarterly scans with a company called BodySpec. Their whole thing is making things cheaper by having repeat visits; a quantity discount, if you will.

Before I decide to do this (and while I wait to hear back about if I can get one done for free with my health plan), I just wanted to get people’s opinions on them. Have you had one or a series done? And more importantly, how has it empowered you to improve your health?

In all honesty I’m not sure the results will encourage me to make any particular change in my lifestyle or routine that I wouldn’t have been able to figure out without it.

Sorry this is pretty rant-y, I tried I promise lol:

I've posted on here re: topics similar to & leading towards this one, but not in this group specifically, so apologies if this is not the right place but idk where else to put it lol: I need to talk to someone who started life less-well off, whatever that means where you come from, and worked themselves into a better situation, and then actually finally broke through whatever the fuck this is that I can't get past right now.

I am solidly working/lower middle class (low low, like I'm not gonna lose my house but I also could never move, no savings but only overdraft -occasionally-, etc.): education background, higher ed, public service, etc—I got too many degrees & got on some good meds & found out mid-30s I have a lot of marketable transferable high-level skills, setting me on a course to replace all the side gigs with one Good Job. Almost got a couple of -really- good offers (like, verbal, then waiting, then layoffs, then a very disappointing email, rinse and repeat) as it unfortunately just happened to be the worst year to look for a job since 2008 lol.

One of the things I'm good at is talking/listening to people, and one of the things I was lacking in was a good network (the other BA/education grads I knew also don't have very good jobs), so I started talking to anyone & everyone, mostly aspirational/mentor types, they have all done very well for themselves & I think it's self-selectinf but they're all pretty sales-y, which is fine. And as another skill I can usually "spot" people, pattern recognition or maybe autism idk, so I'm only talking to people who have had success but also that I don't anticipate will be telling me to pull myself up by my bootstraps, etc.

So here's the thing: every single one of them has said basically (and I think genuinely trying to be helpful): "Why do you need a better job?", "What do you really want here?", "I had what you're after and it didn't make me happy, it was what you already have that did that for me finally," and so if you are having thoughts like that, thank you for reading, please leave lol, because I just cannot—like of course it's easy for you to say "Oh, a corporate job is just travel & restaurants, that's no way to live" when you've already done it! I want in, dude. And honestly it feels like something a woman in the 80s or 90s would have been up against—like "Oh you're better off where you are," ok sure let's fucking swap then. Jesus.

Ok sorry /rant. So what I need is for someone who grew up pretty low income to tell me that there's even a hope, a shred of a possibility that I can do better for myself & my family, because all I can find is people telling me I should be happy with what I have, and the truth is I am not, I'm spinning my wheels and going insane, I am running at maybe 10% capacity and it's driving me nuts—i can literally hear it now "boy I wish I had that much free time”: yeah? What would you do with it? Expensive shit, I bet. See? I feel like I could have both things, but no one wants to let me in—is it just in-group mentality & self-preservstion? Am I somehow threatening their status quo by being a real life person who could maybe do what they did? Because the closest I've gotten so far is hearing how someone along the way cut them a break & that's how they got their start, except none of them seem to be able to see the irony in saying that to me before telling me to just enjoy it as it comes, take it easy, my god I am going to have an aneurysm. And we're talking a significant sample of probably 15-20 people—is it just boomers? Am I doomed?

The most recent take was: "Well you seem like a person who's comfortable and middle-class, so it'll happen, you're fine." And I'm not, like, giving them the hard sell or asking for a job, I'm just mostly listening. I don't get it, and I don't know what I'm supposed to do when I'm trying to find out how someone got to where they are & they end with "You don't need what I have." Is social mobility even a thing anymore in the US?

Edit: gosh I just want to say how much I love this weird lil site, I am feeling about a million times better than I did this time yesterday, to the point that when the person I spoke with (the one who sent me spiraling & considering just going to bed & not getting out of it lol) messaged me to ask if I'd like to talk some more, I didn't say anything but "I'd love to!" because I didn't have to. Because they don't have to get it, they just have to help me find a job—seems like quite a few of you get it & that's all I think I really need sometimes.

(unless you're hiring then pls lmk lol)

Something I'm wondering about: since COVID, I just don't seem to get colds anymore.

Before COVID, I recall that I very regularly had some kind of minor cold – it felt like it was about 50% of the time, realistically let's say maybe 20%? But of course that's just my recollection.

I then caught the original strain of COVID in March 2020, and suffered fairly badly from it for 3 weeks – definitely the worst illness of my life, though I was very lucky not to need to go into hospital. Since then I've had 3 doses of the COVID vaccine, and had COVID at least twice more, both times very mildly.

Since then though, I've noticed that I almost never have any sort of cold any more. I think my partner and I had some kind of brief minor flu back in September, but I've not had anything since, and I remember thinking that it had been a long time since I'd had anything like that.

For reference, I don't wear a mask nowadays, or otherwise do anything much different to my life before COVID.

Does anyone have any thoughts on why this might be?

      Uneducated theories on my part:

      1. Somehow having COVID made me less likely to catch colds
      2. Somehow having the COVID vaccine made me less likely to catch colds
      3. During COVID I got used to holding my breath around people coughing, and that's surprisingly effective towards not catching colds
      4. I'm now mostly around people less likely to pass on colds for some reason

I would love to hear your thoughts on this, especially those who have some actual knowledge about this topic!

My wife and I are having a baby just 1.5 months from now (hooray!). And our insurance provider, Anthem Blue Cross, is cutting coverage to our local hospital network and maternity services in 10 days (boooo!). The entire process of finding out about this (via the news, not our insurance or the hospital) and getting continued coverage has been an absolute nightmare. We jumped the hoops, sent in all the required paperwork, and even got the billing department at the hospital involved. We're still only covered if we happen to be lucky enough that the doctor who is named on the continued coverage agreement happens to be on call at the time of delivery, otherwise it'll be out of pocket to the tune of $10,000 of dollars. At this point it feels like we're betting it all on red.

The response to the United Healthcare shooting illustrated just how frustrated people of the US are in their healthcare system and I'd like to do my part to continue to keep that topic front of mind in the American psyche. I've written up a little op-ed on our experience and I was wonder if any Tilderinos have managed to get one published before. Any insight would be very welcome.

After deciding to get back into the gym after a long hiatus, I was shopping around my area and trying to decide between Crunch or LA Fitness, each for about $40/mo, plus steep initiation fees and annual fees. Searching for a way to get a discounted rate, I stumbled across One Pass Select, offered by my health insurer, United Healthcare. While I was fully prepared to dunk on United, because of how often Dr. Glaucomflecken does, this particular member benefit turned out to be pretty great.

One Pass Select essentially functions as a gym aggregator, offering multiple memberships for one flat rate. So rather than paying Crunch or LA Fitness $40/mo and being subjected to their terms/conditions, I pay One Pass Select $36/mo and get access to:

      • Crunch
      • LA Fitness
      • Anytime Fitness
      • Workout Anytime

And a handful of other gyms I haven't signed up for, because they're not convenient to me. Four gym memberships for $36/mo is pretty cool. Plus if I ever want to cancel, I just cancel my One Pass Select membership from their website, instead of having to sacrifice a squirrel on the 8th day of the sabbath under a blood moon while chanting gregorian monk (the level of difficulty it felt like the last time I wanted to cancel a gym membership).

A quick Google indicates that other insurers offer similar programs:

      • UHC has One Pass Select
      • Cigna/BCBS/Kaiser have Active&Fit Direct
      • BCBS also has Fitness Your Way
      • Aetna has GlobalFit

This was a great find for me, that I'd never even heard of before. Sharing it to hopefully benefit someone else, as well!

Despite vaccination, it seems I got the 'flu (not terrible so far).

I have my own non-prescription comforts and remedies for the sore throat, cough, upset stomach, fever, aches, and so on:

      • Ginger, lemon, and honey tea
      • Ricola cough drops, zinc lozenges
      • PeptoBismol or TheraFlu (if I want to be knocked out)
      • Scorching hot shower with eucalyptus or lavender oil
      • Chicken broth with rice and thyme
      • Electric heated mattress pad cranked up to high

There's little peer-reviewed evidence that any of these make a difference in the course of illness. There's marginal data on the effectiveness of ginger as an antinausea remedy, zinc as an immune booster in people who aren't deficient, and eucalyptus, lavender, and thyme oil components as antiseptics/antibiotics/topical anesthetics/cough suppressants. I'll be the first to concede that I practice all these remedies to give me the illusion of control of suffering, and comforting self-care.

I'm curious as to what home remedies others have tried, why, and how effective or comforting you think they've been.

Has a medical professional ever recommended a non-prescription remedy (not counting Tylenol/acetaminophen) or activity to you for a viral or bacterial infection, or told you to stop a home remedy? Did they give a reason why?

Is there a family, folklore, or alternative medicine tradition that you're following?

Does your home remedy make you feel like you're more comfortable and/or in greater control of your health?

Anyway, anybody have any experience with these sorts of things? I went with this one purely because its metal and I don't want to have some ten-use plastic to recycle if this whole thing leads to improvement.

I don't have sleep apnea but I do think my cute button nose could use a little opening. :)

I have been going through trials to see what ADHD medication might be a good fit for me, and as a side effect have amassed a good deal of unused pills. I have like 60 Vyvanse, a whole bunch of Strattera, and with how expensive they are I'd love to pass them off to someone who might be able to use them. Is this a possibility?

      Cheers!

      Hello,

I seem to be in a write it down phase and have gotten to the physical health part.

I'm looking for fun, absurd, dumb, exaggerated, vanity goals related to fitness.

      What I have now is:

      • standing front/back flip
      • cartwheels as a mode of transportation
      • being able to touch my chins with my forehead with straight legs
      • doing 10 handstand push ups
      • running on all fours
      • being a human flag
      • holding my breath under water for 10min
      • walking with a total weight of 150kg for 4hrs in mixed terrain with an average speed of 5km/h
      • jumping up 1m from standing
      • doing the splits (both ways)
      • gripping my forearms with the opposing arm behind my back from top/bottom
      • going down into the bridge from standing and then get up to a handstand

The idea is to have these feats as goals so that I have something to strive for... I have well over 40years to reach them.

Think of something that you would cringe at when shown or talked about, but secretly you think it's kind of cool.

Like that uncle that always has to show off...

My list is mostly gymnastics, but I sure welcome pure strength, endurance or other things too.

A family member of mine recently got a cheap smart watch online. They weren't planning to use it with a phone app for privacy/security reasons. They thought using it without a phone would be fine, since they just wanted it for the time and for the health features like steps, blood pressure, blood oxygen, sleep tracking, etc. and didn't need to see their texts or other notifications on the watch.

Unfortunately, when we tested the watch, we found that basically all of the features required connecting to a shady no-name company's app. You couldn't even set the time! You can't set the time on a watch without an app!

Also, the watch was a bit bulky for their wrist, and they would prefer a slimmer one.

People of Tildes, do you know of any smart watches that allow you to use the health tracking features without ever connecting to a smartphone? If so, are there any that are slimmer instead of being bulky?
